Wellness Activities

 

Each activity within the Amy's Healthy Living Program is aimed at connect you with your health.  Some activities may require a visit with your physician or health care practitioner, while others are may be completed online or submitting proof of completion to Delta TeamCare. Below you will find a description of each wellness activity and what is required in order to earn your Tomato Tokens.

 

 

 

Annual Wellness Exam

 

 

 

 

 

 

A visit to the doctor once per year might sound like a waste of time, especially when you feel well. But, even though you feel well doesn't mean that you're healthy. Getting your annual checkup can detect health issues before they become serious.  During your annual wellness exam, your doctor will check your basic vital signs, such as blood pressure, vision, and weight.  In many cases, your doctor will also order a biometric laboratory test to check your cholesterol, sugar level (glucose) and other important health elements that support overall well-being.

Health Coaching with Delta TeamCare

 

 

 

 

 

 

Health coaching is a program that provides you with the opportunity to speak with a certified, licensed or master degreed professional who have specialized training in helping individuals establish goals aimed at creating a healthier lifestyle and manage a chronic disease. A consultation with a health coach can be valuable in helping you kick a bad habit or start a good one, decipher the results of your annual wellness exam and help you understand the health risks outlined in your LHRA.

Tobacco-Free Affidavit/Smoking Cessation

 

 

 

 

 

 

The Tobacco-Free Affidavit is your acknowledgment of being tobacco-free. This includes avoiding other alternatives to tobacco, such as e-cigarettes, and other inhalant products that can cause irreparable harm to your health. By attesting to your tobacco-free status, you are acknowledging that you have not used tobacco or tobacco-like products (chews, e-cigarettes, etc.) in the past twelve (12) months and plan on not using these products in the future. 

Lifestyle Health Risk Assessment

 

 

 

 

 

 

The Lifestyle Health Risk Assessment (LHRA) is a wellness tool that allows you to learn more about your lifestyle and behavioral risks that may affect the quality of your life. The LHRA promotes health awareness by providing your information on how to avoid health risks and to make better health decisions.  These results can be shared with your physician and/or your health practitioner to create wellness goals and support your overall health needs.

Wellness on Your Terms

 

 

 

 

 

 

Many of us participate in other activities that promote health and wellness – and realizing that each employee has their own definition of wellness, employees can submit proof of Wellness on Your Terms (WOYT) activity that they have done that promotes good health. Some approved activities include, joining a gym, beach clean-up, Earth Day, Breast Cancer Run through Susan G. Komen or Avon, Leukemia and Lymphoma Team in Training, or other activities such as a Zumba class, yoga class, etc.
